Complex Continuing Care Vision
In collaboration with community partners, to have a continuum of care where people with complex continuing care needs can access appropriate
and timely services and make transitions through an
enhanced comprehensive care network that is supportive, coordinated, compassionate, and embraces clear and transparent communication.
The vision is dedicated to creating a tapestry of care where resources are woven together in an integrated manner, seamless and complete, and responsive over time. Inherent in this vision are provisions for care that:
- build meaningful partnerships with clients;
- enhance the quality of life for those we care for;
is holistic in its approach;
- works proactively with clients to expand capacity and preserve independence; and
- build on the strengths of clients, their families and caregivers.
