Consent Form
One consent must be completed per person prior to vaccine being given. Consents for children under the age of 16 must be completed by a parent or guardian.
Information recorded on this document will be entered into the Saskatchewan Immunization Management System (SIMS). SIMS is a secure computerized system designed to record and store your immunization information. Your immunization information can be made available to authorized healthcare providers to assist in the control of the spread of vaccine-preventable diseases.
Clinic Information
General Information
Immunization of high risk population groups identified by the Ministry of Health began the first week in November with additional risk groups added as vaccine became available. Vaccine was made available to the general public on November 25.
Physicians are now able to offer this immunization to their patients through their office. Not all physician offices will be offering this service so be sure to check first.
The mass public immunization sites were completed on December 17. H1N1 vaccine will still be available by appointment on a limited basis after this date.
Vaccine Availability - Second Dose Information
Vaccine will be available until future notice for those still needing H1N1 immunization.
New Information
The Saskatchewan Ministry of Health no longer recommends a second dose of H1N1 for children under 10. One dose will adequately protect your child from H1N1 influenza disease. |
