Who it is for: 

•  Individuals and their support person, living with heart failure.

Goals:

• Encourage self-management and maximize quality of life for patients, families and support persons living with heart failure.

• Education and support to assist patients to manage their symptoms.

• Help avoid Emergency Room visits and hospital admissions.   

Service:

• Provide printed information and phone follow-up to patients identified in hospital post discharge.
• Provide Heart Failure package to others upon request.

Additional:

• Include and welcome family members and caregivers.
• Appropriate community resources are identified.
