Individuals and their support person, living with heart failure.

Goals:    

Encourage self-management and maximize quality of life for patients, families and support persons living with heart failure.

Education and support to assist patients to manage their symptoms.

Help avoid Emergency Room visits and hospital admissions.       

Service:

Provide printed information and phone follow-up to patients identified in hospital post discharge.

Provide Heart Failure package to others on request.

Additional:         

Include and welcome family members and caregivers.

Appropriate community resources are identified.
