Who it is for:
Individuals who have had a TIA, mild stroke, or are at high risk for stroke.
Goals:
To assist persons to learn stroke risk factor management to reduce the risk of a future stroke.
Service:
Participants meet with an interdisciplinary team of healthcare professionals and a group of their peers to discuss risk factors for stroke and learn how they can actively manage those risk factors. The program includes six modules - Introduction, Risk Factors, Nutrition, Physical Activity, Medications, Psychosocial impact.
Additional:
Participants are encouraged to bring a support person. Participants will also be encouraged to participate in the LiveWell Stroke Exercise Program and other appropriate community programs.
