Goals:
The LiveWell Rheumatology Program provides individuals and their families with disease specific information, support and encouragement assisting in the development of a healthy lifestyle. The Rheumatology Nurse clinician will assist in the development of disease specific self-management skills with the overall goal of improvement in your daily functioning and quality of life.
The team works closely with the rheumatologist and family physician to assist individuals to self manage their condition.
Service:
Referrals to other support services can be initiated by the team based on the needs of the patient.
The Nurse Clinician works in collaboration with dietitians and exercise therapists.
