Who it is for:

Adults and Children with HIV (Human Immunodeficiency Virus) and or Hepatitis C.

Goals:

To provide treatment, education, self-management and support for clients with HIV or Hepatitis C and affected significant others (families).

Clinic based care provided by a multidisciplinary team including infectious disease specialists, gastroenterologist, nurse clinicians and pharmacist.
