LiveWell Adult Cystic Fibrosis Program

Who is it for:

Any adults (17 years and up) living with cystic fibrosis (CF).

Goals:

To optimize management and improve the quality of life for individuals living with cystic fibrosis.

Service:

Provide clinics where clients are supported by a multidisciplinary team consisting of a respirologist, nurse coordinator, physiotherapist, dietitian, pharmacist, social worker and genetic counsellor.
Coordination of home IV Therapy as required.
Coordination of care for hospitalized clients.
