Special Care Homes
Special Care Homes
provide a host of programs delivered within 29 facilities with 2,049
beds. Of the 29 facilities, 10 are wholly owned and operated by
the Region and 19 have a contracted relationship with the Region
for the delivery of designated services.
Special Care Homes are specifically designated
to provide a host of services to meet the needs of those with chronic
illness and disability. All facilities focus on providing quality
of care while ensuring residents quality of life is met to the greatest
degree possible. Residents range in age from children to the elderly.
Care and services are provided based on individual resident need
using an interdisciplinary team approach. In addition to 24-hour
care, Special Care Homes provide a variety of other services including:
- Adult day programs
- Rehabilitation and recovery programs
- Palliative care
- Respite services
Geriatric Assessment Program
The Geriatric Assessment Program sees over 200 clients per month,
through seven different program initiatives. Comprehensive geriatric
assessment includes assessment and treatment of:
- Complicated medical issues
- Physical movement
- Cognitive ability
- Functional ability
- The environment
- Social supports
- Mental health
- Medication issues
Comprehensive geriatric assessment is done by an interdisciplinary
team consisting of:
- Physicians, Geriatrician, Psychiatrist
- Nurses with geriatric speciality training
- Occupational Therapists
- Recreational Therapists
- Physical Therapists
- Social Workers
- Pharmacist
- Dietitian
- Speech Language Pathologist
How do I get an assessment?
In order to get in to see us, a physician sends a consult to the
geriatricians at the Geriatric Program Office at Saskatoon
City Hospital.
What does the Geriatric Assessment Program Include?
10 bed Geriatric Assessment Unit
Clients participate in a comprehensive assessment. Clients are
encouraged to be up, dressed, mobile and participating in their
own care. This is to make the most of independence and improve
activity tolerance. A wander guard system allows confused clients
to move unrestrained on the unit.
20 bed Geriatric Rehab Unit (at Parkridge Centre)
As above, independence is promoted! Clients participate in an active
rehabilitation program that includes the therapy session, practicing
on the unit and assessments such as a self medication program.
20 Patient Per Day Hospital
Hours are from 9:30 a.m. to 2:30 p.m. and clients participate
in therapy and groups much like the inpatient units. Clients are
transported to and from the program.
Outpatient Clinic
Clients from Saskatoon Health Region, and throughout
the province, can be seen by the geriatrician. Often referrals
are made from here to other parts of our service.
Interdisciplinary Assessment Clinic
Clients are seen by the whole team, assessed and the results shared
at a family conference. These sessions last a good half day.
Community Assessments
Our visiting home nurse goes out into the Region and assesses
clients in their home setting. The nurse consults with the geriatrician
and refers clients to other parts of the program as necessary.
Seniors Mental Health Partnership
This service includes a psychiatrist who consults for the various
parts of our program. There are nurses who assess and follow a
caseload in the community. An anxiety management group is run
every few weeks.
