About Us
Continuing Care and Seniors’ Health (CC&SH) is comprised of service teams who collaborate with long term care homes and older adults living in the community. The primary focus is to support and enrich the lives of older adults and individuals living in long term care.
Continuing Care is a multidisciplinary team working with long term care homes to support ongoing improvements through a resident-directed care philosophy.
Continuing Care works with all the LTC homes within SHR, providing consultative and facilitative services. Direction, guidance and mentoring are provided to improve and coordinate safe, accessible, efficient, accountable, equitable and effective service.
Seniors’ Health works collaboratively with older adults who live in the community and other partners to support the promotion of health, physical activity, preventing falls and independence of older adults. This is facilitated through the provision of education, program development and support.
Continuing Care and Seniors’ Health works with many stakeholders to provide and expand supportive programs in response to changing needs.

Special Care Homes
Special Care Homes provide a host of programs delivered within 29 homes with 2,049
beds. Of the 29 homes, 10 are wholly owned and operated by
the Region and 19 have a contracted relationship with the Region
for the delivery of designated services.
Special Care Homes are specifically designated
to provide a host of services to meet the needs of those with chronic
illness and disability. All homes focus on providing quality
of care while ensuring residents quality of life is met to the greatest
degree possible. Residents range in age from children to the elderly.
Care and services are provided based on individual resident need
using an interdisciplinary team approach. In addition to 24-hour
care, Special Care Homes provide a variety of other services including:
- Adult day programs
- Rehabilitation and recovery programs
- Palliative care
- Respite services

Restraint Use Reduced
Despite a least restraint use policy being introduced by the Region in 2004, a care home identified more than 50% of residents being physically restrained on a daily basis in 2005.
Using quality indicators developed by an international research collaborative (interRAI) and supported by the Canadian Institute for Health Information (CIHI), the facility assessed individual residents and compared the home to other long-term care facilities.
A shift in care approaches resulted in reduction of restraint use by 40% at the home and 20% across the Region.

Hand Hygiene
Preventing the spread of germs is a joint effort between staff, volunteers, residents, friends and family.
Hand hygiene, a very simple act, is the most important and effective way to prevent the spread of infection or an outbreak from occurring. Hand hygiene not only protects residents, but protects
everyone, including you and your family, from germs.
