Please complete the appropriate requisition below and send it to
the appropriate department. Requisitions can be received by mail
or fax.
Please ensure the following information is on your requisition:
- Complete patient biographical data, including Date of Birth;
- Contact information such as current address and phone number;
- Saskatchewan Health Number;
- Procedure requested;
- Pertinent Clinical History (this will be used to determine priority
rating for scheduling); and
- Nuclear Medicine also requires the patient height, weight and
current medications
