How To Gain Assistance
Referrals to the Rehabilitation Mental Health Services are to be directed
to Mental Health Services Centralized Intake at Ph. No. (306) 655-7950, Fax

Purpose
Our purpose is to enhance the capacity of clients to function independently
in their community and to engage in meaningful, productive activities of
their choice.
Rehab services is also committed to building a community that
is characterized by acceptance and understanding for persons with
chronic mental illness.
Who Do We Serve?
Persons 18 years of age or older who:
- Have suffered a significant loss of social functioning for
a period generally exceeding 18 months as a result of a mental
health disorder, or
- Individuals at risk of becoming chronically disabled by a
mental health disorder, including persons who are acutely ill
or traumatized.
Types Of Problems
Services for people who have severe and persistent mental health illnesses are
emphasized. Some examples of such illnesses are:
- Schizophrenia
- Bipolar Affective Disorder
- Psychotic Disorder
- Delusional Disorder
- Depression
Services Provided
Clients are encouraged to participate in creating and carrying out their care
plans.
- Case Management / Home Care - Individualized
assessment and care planning is offered, with an aim to achieving
identified goals and objectives, especially in the areas of vocational
issues, leisure pursuits and the skills necessary for daily living.
Services may include assistance with medication compliance,
individual support, education, counseling, and advocacy. Case
Managers work with community agencies, programs and other SDH
mental health services to help ensure a coordinated and comprehensive
approach to each clients' unique needs.
- Residential Services - Clients are assisted
in finding safe, appropriate and sustainable housing which can
provide a level of support consistent with their needs.
- Seniors Focus - Provides case management,
education and consultation to seniors who are experiencing difficulties
due to a mental illness.
- Early Intervention Program - Provides a structured
program of early intervention for people with symptoms of psychosis
during their first two years following onset.
- McKerracher Day and Evening Programs - Provides
a structured, time-limited educational program. Topic areas may
include goal setting, social or communication skills, medication
management, illness awareness, education & coping, assertiveness
training, understanding depression and anxiety management.
- Additional Therapies - Client needs are addressed
through a variety of group and individual programs ( i.e., self-care
skills, conversation skills, budgeting skills, creative-expressive
therapy & collective cooking). Clients are offered education
in order to maintain and improve their leisure pursuits in the
community. Fitness & regular opportunities
for social interaction are also available as well as individual
counseling and family support.
Service Providers
Multi-disciplinary teams that deliver accessible community-based services may
include Community Mental Health Nurses, Occupational Therapists, Psychologists,
Recreation Therapists, and Social Workers.
Rehab also works very closely with community organizations such
as the Canadian Mental Health Association, Crisis Management Services,
Crocus Co-op, and the
Saskatoon Housing Coalition.
