This primary health centre was announced in May 1999, to address
identified needs within a highly concentrated seniors' population
in Saskatoon. In April 2001, a clinic opened at the Scott Forget
Housing Authority, a senior citizen’s housing complex.
A nurse practitioner, client care coordinators, and a receptionist/lab
assistant are available Monday to Friday from 8:30 a.m. to 5:00
p.m. Other team members include a community developer, an addictions
counsellor, home care nurse, a public health nurse, clinical

Borden Primary Health Centre
The Borden Primary Health Centre was established March 2000.
Members of the primary health team include a nurse practitioner,
a laboratory technician, a public health nurse, home care nurse,
client care co-ordinator and mental health counsellor. Academic Family Medicine
(University of Saskatchewan) provide physician services at this site. Itinerant

Delisle Primary Health Centre
The Delisle Primary Health Centre was designated as a primary health site on September 1, 2006. Members of the primary health team include a nurse practitioner, physician, Public Health Nurse, Home Care, CPAS, Massage Therapist and a dentist. White Buffalo Youth Lodge
The White Buffalo Youth Lodge is a partnership between Saskatoon
Health Region, Saskatoon Tribal Council, City of Saskatoon, and
Central Urban Metis Federation Inc. The Lodge was officially
opened in December 2000. White Buffalo is a multi-purpose health
and recreation facility located in a core area of Saskatoon which
has a high aboriginal population. The primary health team at
White Buffalo Youth includes a nurse practitioner, addictions
worker, public health staff, and a physician from the West Side
Community Clinic.

Nutana Integrated School-Linked
Services
Nutana Collegiate is a community school providing alternate
choices for youth within the public secondary education program.
A nurse practitioner works together with other support staff
at Nutana, teachers, families, and students to address health-related
questions and concerns. The aim is to help students make healthier
lifestyle choices so they can feel and be healthier in order
to achieve their educational goals.
University of Saskatchewan Student
Health Centre

health services to students at the University of Saskatchewan
Student Health Centre.
Wynyard Community Health Centre
Services at the Wynyard Community Health Centre focus on disease
prevention, health promotion, early intervention, treatment and
follow-up care. A nurse practitioner and four physicians

Strasbourg and District Health Centre
The Strasbourg and District Health Centre is a well established
primary health site. It's primary health team includes
public health staff, a nurse practitioner and a physician.

West Winds Primary Health Centre
West Winds Primary Health Centre opened to the public on April 4, 2006. The site is located at 3311 Fairlight Drive. Academic Family Medicine (University of Saskatchewan) has relocated to the centre and will work in an integrated partnership model with the Saskatoon Health Region.

Duck Lake Primary Health Team
The Duck Lake Primary Health team was designated as a primary health site in the fall of 2007. Members of this primary health team include a nurse practitioner, physician, Public Health Nurse, Community School Coordinator, School Counsellor, Literacy Coordinator and a Dental Nurse. Health Bus
Mobile Primary Health Unit - The Health Bus is an inter-professional Mobile Primary Health Centre based on partnerships including M.D. Ambulance, CUMFI, First Nations agencies, Saskatoon Health Region and the Ministry of Health.
The team of healthcare professionals (including a paramedic and nurse practitioner) focuses on community care. They:
- Establish links with social agencies and programs
- Work with citizens and communities to reach the optimal level of wellness
- Work in a strength-based model with community members
- Are committed to maintaining and promoting health, wellness and injury prevention
- Serve as an advocate, facilitator, liaison and resource
coordinator
- Identify challenges that may interfere with the development of a better, more integrated health care system

A Nurse Practitioner (NP) is a Registered Nurse who works in a Primary Health setting, has additional education and training, writes a national qualifying exam and is licensed by the Saskatchewan Registered Nurses Association as an independent practitioner.
