If you would like a copy of your immunization record, complete the Immunization Record Request Form and fax to 306-655-4711 or email to phsrecordline@saskatoonhealthregion.ca You may also complete a record request form at our North East office, #108-407 Ludlow Street.

There is a $25 administrative fee for records provided to persons over 16 years of age.

In order to provide you with the most recent copy of your immunization record, Population and Public Health searches for your record in the electronic provincial immunization registry called Panorama.  If a hard copy record is also located, it will be back entered into this system.  Records are stored in this registry if you have lived in or attended school within Saskatoon Health Region.

A record of immunization from the provincial immunization registry will be provided for clients, physicians, and public health offices within 7 days of client contacting Population and Public Health. 

Personal health information shall not be released to any other person other than the individual or a parent/guardian of children under 18 years.

A parent/guardian cannot request an immunization record for a 'child' 18 years of age and older without written or verbal consent being provided to Population and Public Health.

Clients may give permission verbally, in writing or by email for someone else to pick the record up.

Personal health information, including immunization histories, can be mailed, faxed or picked up by the client.  Personal health information is faxed with a confidential notice, identifying sender on the fax cover sheet and intended recipient number.
