​Buccal Swabs for HLA Typing

Sample Collection Instruction

You have been asked to provide a sample of cheek (buccal) cells to perform a DNA test in the laboratory. It is important that the attached instruction sheet be followed exactly to provide the best specimen.

Four swabs are required. Use two swabs per cheek.

Place sealed and labelled tubes into a cushioned envelope, along with the accompanying requisition, and send to:

St Paul's Hospital
1702 20th Street West
Saskatoon, SK Canada S7M 0Z9

Indicate on the outside of the envelope "Exempt Human Specimen"

Be sure to include the accompanying requisition. The requisition must be signed by the person collecting the swabs, and the date and time of collection must be filled out.

The cost of mailing is at your expense..
