Forms Required

  • Patient weight (kg), address, home phone number and reason for use must be indicated
  • If recipient is a neonate, indicate mother’s first and last name
  • Saskatoon laboratory only:
    • TIG/HBIG/Hepatitis B/RIG/Rabies Vaccine Recipients Form (DC-101) shall be completed and faxed to Disease Control Public Health Services (PHS) when product issued
    • Indication of PHS notification shall be made in Transfusion Medicine LIS
  • Saskatoon nursing staff only:
    • Record of Administration of High Risk Vaccine/or Immune Globulin Form (DC-264) shall be completed at the time of administration of vaccine and faxed to 655-4723. Form shall accompany the product from Transfusion Medicine
  • A Transfusion Medicine/Canadian Blood Services chart report is NOT required for product infusion
