​Ordering Recommendation
  • ​​Collect blood between 7:00 a.m. and 10:00 a.m.
  • Both aldosterone and direct renin levels and ratios will be determined

  • ​Plasma
  • Preferred Collection Container: Lavender (EDTA)
  • Alternative Collection Container: None
​Collection Procedure
  • Collect and spin at room temperature, aliquot as soon as possible and freeze
  • if specimen thaws, it is unsuitable for analysis
​Required Volume
  • ​Optimal Volume: 4 mL Full collection tube
  • Minimum Volume: 2 mL plasma
    (Submitting the minimum volume makes it impossible to repeat the test or perform confirmatory/reflex testing. In some situations, a minimum volume may require a second collection.)

  • Reference Range for Renin (Direct):
    • Upright Posture: <55 ng/L
    • Supine Posture: <24 ng/L
    • Reference ranges provided are based on the 5th-95th percentile
  • Aldosterone/Renin Ratio:
    • <=100: suggests essential hypertension
    • 101-139: repeat test suggested
    • ​>= 140: suggests primary aldosteronism
  • All reference ranges assume normokalemic ambulatory patients with normal salt intake, not on hypertension medication
  • The testing results are affected by posture, sodium intake, diuretics and hypertensive medication
