Forms Required
  • Saskatoon lab only:
    • Release of HBIG/Hepatitis B Vaccine/RIG/Rabies Vaccine/TIG Form (DC-101) shall be completed and faxed to Disease Control Public Health Services (PHS) when product issued. Indication of PHS notification shall be made in Transfusion Medicine LIS.
  • Saskatoon nursing staff only:
    • Record of Administration of High Risk Vaccine/or Immune Globulin Form (DC-264) shall be completed at the time of administration of vaccine and faxed to 655-4723. Form DC-87 shall be completed by nursing staff at the time of administration.   These forms shall accompany the product from Transfusion Medicine.
  • A Transfusion Medicine/Canadian Blood Services chart report is NOT required for product infusion
