Who Do We Serve?
Not qualifying for other resources in MHAS and community.
Presentation of severe and persistent mental illness with addictions comorbidity.
Needing intensive stabilization through an interdisciplinary team of nursing, AC, SW, and OT. Will require significant resources to participate in the community.
Low engagement/ difficult to serve.
History of difficulty with voluntary participation in treatment strategies and support programs.
History of failed MHAS interventions.
Low socio-economic status complicating client presentation. Connected to little or no professional and family supports.
Frequent emergency department visits
COAST team utilizing cross-functional approaches to assist in stabilizing individuals with severe and persistent mental health and substance use disorders in the community.
Intensive support in stabilization and connection to longer term community resources and services, which may include medium to longer term case management.
The program includes support clients transitioning to the community from hospital, residential treatment, or detox; recovery support; intensive case management and crisis intervention.
Our multidisciplinary team consists of Addiction Counsellors, Social Workers and Community Mental Health Nurses.
